Service Description
Consultation Fee is applied toward your first month of coaching. Review of current training and lifestyle behaviors. Assessment of goals and determination of appropriate membership package. PDF notes and onboarding. New clients, you will need to fill out a running history form available on our site under FORMS. Returning clients, you will need to fill out a health revisit form available on our site under FORMS. Cancellation Policy
New clients booking running services: You must fill out the Running History Form. Returning clients, you may fill out Health Revisit Form. New clients booking health services or personal training: You must fill out the Health History Form and the PAR-Q form. New clients booking Oasis Recovery services must fill out the Oasis Liability Waiver. All forms can be found on our website: https://www.magnumopuscoaching.com/forms Please note that virtual scheduling is only available for remote clients. All other clients will be in-person. We will work to accommodate changing schedules, but no refunds will be given for prescheduled and/or purchased services, and we will endeavor to reschedule as the time and calendar allow.
